Job summary

Centene Corporation is seeking a licensed behavioral health professional for a fully remote role in New York. The position centers on telephonic case management, coordinating complex care plans for high-acuity members, and educating families on benefits and services.

Requires a Master’s degree in Behavioral Health or Social Work (or Nursing) and 4–6 years of related experience, with NY licensure (LCSW/LMSW/LMFT/LMHC or RN) per contract.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or Nursing plus 4–6 years of related experience.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN depending on state contract requirements.
• Behavioral health experience is required; children and families experience preferred.

Responsibilities

• Develops, assesses, plans, implements, and coordinates complex care management for behavioral health and social determinants.
• Provides telephonic case management and outreach for high-risk members.
• Educates members and families on services, benefits, and care options.
• Coordinates between members, families, and care teams to ensure timely access to services.
• Maintains high-acuity caseloads and monitors outcomes to adjust care plans.
• Collaborates with healthcare providers and community resources to optimize care.
• Precepts new clinical hires and supports onboarding and readiness.
