Job summary

Centene Corp. in Kansas is seeking a remote Behavioral Health Case Manager to develop and coordinate complex care management for members with mental health and substance use needs.

The role requires an active licensed MH professional (LCSW, LMSW, LMFT, LMHC, or LPC) or RN with BH experience, a master’s degree, and 2–4 years of related work experience. The position offers a Monday–Friday 8:00 AM–5:00 PM schedule and opportunities to collaborate with multidisciplinary teams.

Qualifications

• Requires a Master’s degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2–4 years of related experience.
• Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experience required

Responsibilities

• Evaluates the needs of the member via phone or in-home visits related to resources and recommends the care plan/service plan.
• May perform telephonic, digital, home and/or other site visits outreach to assess member needs and collaborate with resources
• Develops ongoing care plans for high-acuity members and identifies providers, specialists, and community resources for BH needs
• Coordinates between member/family/caregivers, community resources, and care provider team to ensure services are accessible
• Monitors care plans and member status for changes and provides recommendations for adjustments
• Facilitates care coordination to ensure timely access to needed care or services
• Collects and maintains member information to ensure regulatory compliance
• Provides education to members and families on procedures, options, referrals, and benefits
• Provides leadership with feedback on opportunities to improve care delivery
• Performs other duties as assigned
