Qualifications

• Requires a Master’s degree in Mental Health or Social Work or RN with behavioral health experience.
• Licensed Behavioral Health Professional or RN as per state contract requirements: LCSW, LMFT, LMHC, LPC, or RN with behavioral health experience.

Responsibilities

• Evaluate needs of complex, high-risk members with mental/behavioral health conditions and develop care plans.
• Act as liaison between member, family, physician, and facilities.
• Perform frequent home or site visits to assess needs and coordinate resources.
• Educate members and families on preventive care, services, and treatments.
• Coordinate community resources and service authorizations for various needs.
