• Coordinates between members, families, resources, and care teams to ensure timely access to services.
• Monitors plan status and outcomes; provides care plan recommendations based on member needs.
• Educates members and families on procedures, treatments, and benefits; supports quality of care improvements.

Education

Master's degree in Behavioral Health or Social Work

Nursing degree from Accredited School of Nursing
