Qualifications

• 2–4 years of related experience required.

Responsibilities

• Evaluate member needs via phone or in-home visits and support care plans.
• Outreach to assess needs and collaborate with resources.
• Develop and update care plans for high-acuity members.
• Coordinate between members, families, providers, and resources.
• Monitor status and outcomes, adjusting care plans as needed.
• Document member information for regulatory compliance.
• Educate members and families on procedures and benefits.
• Provide feedback to leadership on care delivery improvements.
• Perform other duties as assigned.
• Adhere to all policies and standards.
