Job summary

Centene Corp. is seeking a remote Care Manager for Georgia residents to develop and coordinate complex care plans for members with mental health and substance use needs.

The role requires an active RN license or a Licensed Master’s Behavioral Health Professional credential (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) and 2–4 years of related experience. Telephonic engagement and strong communication skills are essential.

Qualifications

• Master's degree in Behavioral Health or Nursing degree required.
• Active licensure as RN or LCSW/LMSW/LMFT/LMHC/LPC as per state contract requirements.
• 2–4 years of related experience in care management or behavioral health.

Responsibilities

• Develops and facilitates complex care plans for behavioral health needs.
• Coordinates with members, families, and care providers to ensure access to services.
• Monitors outcomes and makes recommendations to improve care and cost-effectiveness.
• Educates members and families on procedures, treatment options, and benefits.
• Performs outreach via telephonic, digital, or in-home visits as needed.
