Job summary

Centene Corporation is seeking a remote Care Manager in Georgia focused on mental health and substance use care coordination. The role requires an active RN license or a Licensed Master’s Behavioral Health Professional credential and 2–4 years of related experience.

The ideal candidate will have strong telephonic communication, coordination skills, and the ability to assess member needs and develop comprehensive care plans, while complying with regulatory requirements.

Qualifications

• Master's degree in Behavioral Health or Social Work or nursing degree required
• Active RN license or Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, or LPC) per state contract
• 2–4 years of related experience
• Must reside in Georgia and hold Georgia license/credential

Responsibilities

• Evaluate member needs by phone or home visits and develop care/service plans.
• Conduct telephonic, digital, home outreach to assess needs.
• Develop care plans for high-acuity members and coordinate providers.
• Coordinate between member, family, resources, and care team to ensure services are accessible.
• Monitor care plans and outcomes; adjust as needed.
• Facilitate care coordination to ensure timely access to services.
• Collect and maintain member information per regulations.
• Educate members and families on procedures and benefits.
• Provide leadership with feedback to improve care and cost-effectiveness.
• Perform other duties; comply with policies.
