Qualifications

• Master's degree in Behavioral Health, Social Work or Nursing.
• CA license in BH field or RN license is required by state contract.
• 2–4 years of related experience in care management or BH required.

Responsibilities

• Evaluate member needs via phone or in-home visits and develop care/service plans.
• Coordinate between members, families, community resources, and care teams to ensure access to services.
• Develop ongoing care plans for high-acuity members and monitor status for changes in treatment.
• Educate members and families on healthcare procedures and benefits and ensure compliance.
