Responsibilities

• Assess member needs by phone or in-home visits and develop care plans.
• Coordinate between member/family and providers to ensure access to services.
• Provide education to members and families on procedures and benefits.
• Monitor care plans and adjust based on outcomes.
• Perform outreach via telephonic, digital, home or site visits.
• Ensure compliance with state, federal, and payer regulations.
