Centene Corp. is seeking a Senior Care Manager to develop and coordinate complex care management for behavioral health needs, primarily via telephonic outreach. The role is fully remote with a Monday–Friday schedule 8:30 a.m.

to 5:00 p.m. ET and requires NY licensure (LMSW/LCSW/LMFT/LMHC/LPC or RN) and 4–6 years of related experience. The person will create personalized care plans, identify providers and community resources, monitor outcomes, and educate members and families on benefits.

Qualifications

• Master's degree in Behavioral Health or Social Work.
• Nursing degree from an accredited school may be required.
• 4–6 years of related experience in behavioral health or care management.

Responsabilités

• Develops and coordinates ongoing care plans via phone or in-home visits for high-acuity members.
• Identifies providers and community resources needed for care, including mental health and social determinants.
• Maintains a high-volume caseload of complex/specialty cases with strong clinical judgment.
• Collaborates with healthcare providers to ensure timely access to needed services.
• Educates members and families on procedures, treatment options, and benefits.
• Ensures compliance with state, federal, and payer regulations.
