Job summary

Centene Corporation seeks a remote Care Manager focused on behavioral health to develop, coordinate, and educate members on services and benefits. You will conduct telephonic case management, collaborate with providers, and ensure timely access to care for high-need individuals.

Candidates must be licensed in New York (LMSW/LCSW/LMHC/LMFT/LPC or RN), hold a master’s in behavioral health or social work (or nursing), and have 4–6 years of related experience. Mon–Fri 8:30 a.m.–5:00 p.m.

Qualifications

• Master's degree in Behavioral Health or Social Work or a degree from an accredited school of nursing with 4–6 years of related experience

Responsibilities

• Develops and coordinates complex care plans for high-acuity members via telephonic outreach and/or home visits
• Identifies providers, specialists, and community resources for care including mental health and social determinants
• Maintains a high-volume caseload of complex/specialty cases and monitors outcomes
• Collaborates with healthcare providers to ensure timely access to needed care or services
• Educates members and families on procedures, options, and benefits, including behavioral health needs
