Job summary

Centene Management Company LLC is seeking a telephonic care manager focused on behavioral health for a fully remote role. You will assess needs, develop care plans, and coordinate with providers to ensure high‑quality, cost‑effective outcomes.

Licensed BH professional or RN licensure is required by state contract. The position requires a master’s degree in behavioral health or social work (or nursing) and 4–6 years of related experience.

Qualifications

Requires Master’s degree in Behavioral Health or Social Work or a nursing degree.
4–6 years of related experience.
License required: LCSW/LMSW/LMFT/LMHC/LPC or RN depending on contract requirements.
Behavioral health experience is required; pediatric experience preferred.

Responsibilities

Provide telephonic case management and outreach for high‑risk members with complex behavioral health needs.
Develop and adjust care plans/service plans and educate members and families on services and benefits.
Identify providers, specialists, and community resources to support care and address mental health and social determinants.
Coordinate between member/family and care team to ensure timely access to services and supports.
Monitor member status, track outcomes, and revise plans based on changes in treatment and symptoms.
