Qualifications

• Master's degree in Behavioral Health, Social Work, or Nursing.
• 2–4 years of related experience.
• Active WA state licensure and/or Compact RN licensure.

Responsibilities

• Evaluates the needs of the member via phone or in-home visits and recommends care/service plans for optimal outcomes.
• Performs telephonic, digital, home and/or other site visits to assess member needs.
• Develops ongoing care plans for high-acuity members and coordinates providers and resources.
• Coordinates between member/family, community resources, and care providers to ensure access.
• Monitors care plans and outcomes and advises on changes as needed.
• Facilitates care coordination to ensure timely access to services.
• Collects and maintains member information to ensure regulatory compliance.
• Provides education to members and families on procedures, options, and benefits.
• Provides feedback to leadership to improve care delivery.
• Performs other duties as assigned and adheres to policies.
