Job summary

Qualifications

• Master's degree in Behavioral Health or Social Work or Nursing degree with BH experience.
• 2–4 years of related experience in behavioral health or care management.
• Licensed Behavioral Health Professional or RN with BH experience as per state requirements.

Responsibilities

• Develops, assesses, and facilitates complex care management activities for behavioral health needs.
• Evaluates member needs via phone or home visits and coordinates care plans.
• Collaborates with providers and resources to ensure timely access to services.
• Monitors care plans and modifies them based on outcomes and symptoms.
• Educates members and families on procedures, treatment options, and benefits.
• Ensures compliance with state, federal, and payer regulations.
