Job summary

Centene is seeking a Care Manager (Behavioral Health) to develop, assess, and facilitate complex care management for members with mental and behavioral health needs. This role emphasizes high-quality, cost-effective outcomes, delivered through remote and in-person engagement.

Responsibilities include evaluating member needs via phone or home visits, coordinating care plans, educating members and families, and collaborating with providers.

Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing.
• 2–4 years of related experience.
• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC, LAC) or RN based on state contract requirements with BH experience - required

Responsibilities

• Evaluate the needs of the member via phone or in-home visits and recommend/facilitate care plans.
• Perform telephonic, digital, home, and/or other site visits outreach to assess member needs.
• Develop ongoing care plans for members with high level acuity and identify necessary providers and resources.
• Coordinate between the member, family/caregivers, community resources, and care provider team.
• Monitor care plans/service plans and member status for changes in treatment and provide recommendations.
• Facilitate care coordination and collaborate with appropriate providers or specialists.
• Collect, document, and maintain member information and care management activities.
• Provide education to members and their families on procedures, treatment options, and healthcare benefits.
• Provide feedback to leadership on opportunities to improve care and quality delivery.
• Perform other duties as assigned and comply with all policies and standards
