Qualifications

• Must reside in Arizona for AZ Health Plan eligibility.
• Licensure as BH professional (e.g., LCSW/LMSW/LMFT/LMHC/LPC/LAC) or RN is required for AZ Health Plan.

Responsibilities

• Assesss member needs via phone or home visits and coordinate care plans.
• Provide education to members and families on procedures and benefits.
• Coordinate with providers to ensure timely access to services.
• Document member information and care activities for compliance with regulations.
• Support care management for behavioral health and substance use disorders.
