Job summary

Centene is seeking a fully remote Behavioral Health Case Manager to support Kentucky members with complex care needs. You will conduct assessments via phone or in-home when needed and coordinate services to improve outcomes. Requires a Master's degree in Behavioral Health or Social Work or a Nursing degree with BH experience, and 2–4 years in case management.

Active Kentucky licensure (LCSW/LPCC) is preferred. Schedule is Monday–Friday, 8:00 AM–5:00 PM Eastern.

Qualifications

Requires Master's degree in Behavioral Health or Social Work or RN with BH experience.
2–4 years of related case management experience.
Active Kentucky licensure preferred (LCSW/LPCC).

Responsibilities

Evaluates member needs via phone to develop care plans for behavioral health and SMI/substance use needs.
Performs outreach to coordinate services and supports.
Develops ongoing care plans for high-acuity members with providers and community resources.
Coordinates between members, families, resources, and care teams for access to services.
Monitors plans and outcomes, adjusting as needed.
Provides education to members and families on procedures, options, and benefits.
Provides feedback to leadership for quality improvements.
