Job summary

Centene is seeking a remote Behavioral Health Case Manager to develop and coordinate care plans for members with complex behavioral health needs, including SMI and substance use disorders. You will assess needs via phone or home visits and connect members with providers, resources, and education to improve outcomes.

Qualified candidates have a Master’s degree in Behavioral Health or Social Work (or Nursing with BH experience) and an active licensure (LCSW/LPCC/LMSW/ LMHC/LPC).

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or a Nursing degree with BH experience.
• Active licensure as LCSW, LMSW, LMFT, LMHC, LPC or RN per state contract requirements.
• 2–4 years of related case management and mental health services experience.

Responsibilities

• Develops and coordinates complex care plans for behavioral health needs.
• Assesses member needs via phone or home visits and connects to providers.
• Collaborates with resources to ensure timely access to care and services.
• Educates members and families about treatment options and benefits.
• Monitors plans and outcomes, adjusting care as needed.
