The role requires a Master’s degree in Behavioral Health or Social Work or Nursing, plus 2–4 years of related experience and active licensure (LCSW/LMSW/LMFT/LMHC or RN per state needs).

Qualifications

• Requires Master’s degree in Behavioral Health or Social Work or Nursing.
• Active licensure preferred (LCSW/LMSW/LMFT/LMHC or RN as per state requirements).
• 2–4 years of related experience in case management or behavioral health services.

Responsibilities

• Develops and coordinates comprehensive care plans for members with complex behavioral health needs.
• Performs outreach via phone, telehealth, or in-home visits to assess needs and resources.
• Monitors and adjusts care plans based on patient progress and provider input.
• Educates members and families about procedures, treatment options, and benefits.
