Colorado Department of Human Services is seeking a Clinical Care Coordinator to work remotely from home with travel for in-person meetings in Colorado. The role focuses on coordinating behavioral health care, case management, and placement for individuals with serious mental illness in jail and hospital settings.

The successful candidate will hold Colorado licensure (LCSW/LPC/LMFT/LAC), have 1+ year of licensed experience, and ideally 2+ years in behavioral health.

Formación

• Current, valid Colorado licensure as a Licensed Clinical Social Worker (LCSW) OR Licensed Professional Counselor (LPC) OR Licensed Marriage and Family Therapist (LMFT) OR Licensed Addiction Counselor (LAC).
• At least 1 year of licensed experience in direct care behavioral/mental health treatment.
• Preferred qualifications include 2+ years of behavioral health experience.
• Bilingual in Spanish strongly preferred.
• Located within 45 miles of 81501.

Responsabilidades

• Provide therapeutic interventions, including psychosocial therapies and clinical assessments within scope of practice and licensure.
• Individualize psychosocial therapy approaches to meet patient needs.
• Provide training and consultation to staff and participate in care plans.
• Interpret assessment results and apply collateral information to identify necessary services.
• Coordinate access to treatment services, including medication evaluation and case management.
• Maintain comprehensive documentation and ensure privacy compliance in the EHR.
• Collaborate with Care Coordination Program and stakeholders to coordinate care and placements.
• Act as clinical expert and liaison between healthcare and behavioral health systems, courts, and community.
