Job summary

Colorado Department of Human Services (CDHS) is seeking a Clinical Care Coordinator to provide therapeutic interventions and comprehensive care coordination for civil and forensic mental health clients. Qualifications

• Current Colorado licensure in one of the listed clinical disciplines (LCSW, LPC, LMFT or LAC).
• Minimum of one year of licensed direct care behavioral/mental health experience.

Responsibilities

• Provide therapeutic interventions including psychosocial therapies and evidence-based clinical assessments.
• Coordinate care and develop case plans for patients in jail and hospital settings.
• Train and consult with other staff; participate in individual patient plans of care when needed.
• Maintain detailed documentation of client interactions and care outcomes.
• Collaborate with Care Coordination Program and external stakeholders to ensure continuity of care and appropriate placements.
