State of Colorado is seeking a Clinical Care Coordinator for Civil and Forensic Mental Health Services. The role supports patients with serious mental illness, coordinating care across jail and hospital settings and ensuring appropriate placements and services.

It requires active Colorado licensure and one year of licensed experience, with Spanish language skills strongly preferred. Position is remote with travel for in-person meetings, primarily in the Denver metro area.

Formación

• Current, valid Colorado licensure as an LCSW, LPC, LMFT, or LAC.
• At least one year of licensed direct care behavioral/mental health experience.
• Bilingual in Spanish strongly preferred.
• Located within 40 miles of 80236.
• Must pass TB screen and background checks as required by the state.

Responsabilidades

• Provide therapeutic interventions and clinical care coordination for patients with serious mental illness in jail and hospital settings.
• Individually tailor psychosocial therapy approaches to meet patient needs.
• Provide training and consultation to other professional staff and participate in individual patient plans of care when needed.
• Interpret and apply clinical assessment results to identify required treatment or social services.
• Coordinate access to medication evaluation, mental health treatment, substance abuse treatment, and related services.
• Maintain communication with Care Coordination Program, jails, hospitals, courts, and community partners to ensure continuity of care.
