Job summary

CareStar, Inc. is seeking an Assessment Specialist to join the Specialized Recovery Support Services program serving Fayette County, Ohio and surrounding areas.

This full-time role combines remote work with required field visits to assess client needs, develop personalized care plans, and connect individuals with essential services and supports. You will work with a mission-driven team, maintain confidentiality, and stay informed on Medicaid and community resources to support diverse

Qualifications

• Bachelor's degree in social work, counseling, psychology, or related field; or RN/LSW.
• Minimum of one year post-degree experience with chronic conditions and/or three years post-degree with severe mental illness.
• Knowledge of Medicaid, behavioral health systems, and community resources.
• Effective interpersonal (oral and written) skills with diverse populations.

Responsibilities

• Maintains knowledge of State and program requirements.
• Completes in-home visits to assess eligibility and conducts safety assessments.
• Documents all contacts and interventions in the applicant's record per policy.
