Job summary

CareStar, Inc. is seeking an Assessment Specialist for Warren County, Ohio and surrounding areas. This full-time role combines remote work with periodic in-home visits to assess eligibility and develop care plans for clients with chronic conditions or severe mental illness.

You will work within the Specialized Recovery Support Services program, coordinating with healthcare resources and community services to improve outcomes and independence for participants.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or related field; or RN, or LSW.
• One-year post-degree experience with individuals with chronic conditions and/or three years post-degree with severe mental illness.
• Knowledge of Medicaid, behavioral health systems, and local community resources.
• Strong interpersonal (oral and written) skills to interact with diverse populations.

Responsibilities

• Maintains and builds knowledge of State and program requirements.
• Maintains knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
• Performs in-home visits to assess eligibility and conducts safety assessments.
• Documents all applicant and case-related contacts in the record per policy.
• Provides assistance with community resource referrals as appropriate.
• Adheres to CareStar policy and procedure.
• Maintains patient confidentiality and follows Acceptable Use Policy.
