Job summary

CareStar, Inc. seeks an Assessment Specialist to serve Marion County, Ohio, and surrounding areas. You will assess needs, develop personalized care plans, and connect clients with essential services while working remotely with field visits to their homes as required.

Required qualifications include a social work-related degree or RN/LSW and experience with chronic conditions or severe mental illness; strong communication and knowledge of Medicaid and community resources are essential.

Qualifications

• Bachelor's Degree in social work, counseling, psychology, or related field; or RN or LSW.
• Minimum one-year post-degree experience with chronic conditions and/or three years post-degree with severe mental illness.
• Knowledge of issues affecting people with severe mental illness and community resources.
• Strong interpersonal (oral and written) skills across diverse populations.
• Experience with chronic conditions or severe mental illness

Responsibilities

• Maintains and updates knowledge of state and program requirements.
• Understanding Medicare, third-party payers, community resources, and ADA.
• Performs in-home visits to assess eligibility and safety per guidelines.
• Documents all contacts and interventions in the applicant's record.
• Assists with community resource referrals as appropriate.
• Adheres to CareStar policies and procedures.
• Maintains patient confidentiality; reports any breaches.
• Complies with the Acceptable Use Policy for CareStar information systems.
