Job summary

CareStar, Inc. seeks an Assessment Specialist to join the Specialized Recovery Support Services team in Ohio. You will assess client needs, develop personalized care plans, and connect individuals with essential services, including in-home visits as needed.

You will work with diverse populations, applying knowledge of Medicaid and behavioral health systems while maintaining confidentiality. This is a full-time, remote role with field visits across Shelby County and surrounding areas.

Qualifications

• Bachelor's degree or RN/LSW credential required.
• Experience with chronic conditions and/or mental illness preferred.
• Strong interpersonal and written communication skills.

Responsibilities

• Maintains knowledge of state and program requirements.
• Knowledge of Medicare, third-party payers, and community resources.
• Conducts in-home visits to assess eligibility and safety.
• Documents all contacts and interventions in client records per policy.
• Assists with community resource referrals as appropriate.
• Adheres to CareStar policies and procedures.
• Maintains patient confidentiality and complies with acceptable use policies.
