Formación

• Bachelor's degree in social work, counseling, psychology, or a related field.
• Registered Nurse (RN) or Licensed Social Worker (LSW) credentials.
• Knowledge of Medicaid and behavioral health systems.

Responsabilidades

• Performs in-home visits to assess eligibility and safety.
• Develops personalized care plans and coordinates services.
• Maintains documentation of contacts and interventions per policy.
• Keeps up-to-date with state and program requirements.
