CareStar, Inc. is hiring an Assessment Specialist for the Cuyahoga County area and surrounding counties. This remote role includes field visits and focuses on assessments, care planning, and linking individuals to essential services.

You will work within the Specialized Recovery Support Services program to help clients live more independent lives. The ideal candidate holds a bachelor’s in social work, counseling, psychology, or a related field, or is an RN/LSW, with experience in chronic

Qualifications

• Bachelor's Degree in social work, counseling, psychology, or related field; or RN/LSW.
• 1+ year post-degree experience with chronic conditions and/or 3+ years post-degree experience with severe mental illness.
• Knowledge of issues affecting people with severe mental illness and community-based interventions/resources.
• Effective interpersonal (oral and written) skills across diverse populations.

Responsibilities

• Maintains and builds knowledge of State and program requirements.
• Knows Medicare, third-party payers, community resources, and ADA.
• Conducts in-home visits to assess eligibility and safety per guidelines.
• Documents contacts and interventions in the applicant's record promptly and accurately.
• Assists with community resource referrals as appropriate.
• Adheres to CareStar policies and procedures and maintains confidentiality.
• Complies with the Acceptable Use Policy for CareStar information systems.
