Job summary

CareStar, Inc. is seeking an Assessment Specialist in the Hamilton County area (OH) to assess individuals, develop care plans, and connect them with essential services.

Qualifications

• Bachelor's Degree in social work, counseling, psychology, or a related field; or RN or LSW.
• At least one year post-degree experience working with individuals with chronic conditions and/or a minimum of three years’ post-degree experience with individuals with severe and persistent mental illness.
• Knowledge of Medicaid, behavioral health systems, and local community resources.
• Effective interpersonal (oral and written) skills to interact professionally with all age groups from diverse backgrounds.

Responsibilities

• Maintains and builds knowledge of the State and program requirements.
• Maintains and builds on knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
• Completes in-home visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
• Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy.
• Provides assistance with community resource referrals as appropriate.
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality as related to patient information.
