Job summary

CareStar, Inc. is seeking an Assessment Specialist to join our Specialized Recovery Support Services program in Ohio. This role involves in-home assessments, developing care plans, and connecting clients with essential services.

You will collaborate with a mission-driven team, handle Medicaid/behavioral health resources, and work with diverse populations while maintaining confidentiality and professional standards. Remote flexibility with field visits is offered.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or related field or RN/LSW.
• Knowledge of Medicaid, behavioral health systems, and local community resources.
• Experience with chronic conditions or severe mental illness and community-based interventions.

Responsibilities

• Conduct in-home visits to assess eligibility for programs and perform safety assessments.
• Develop personalized care plans and connect individuals with needed services.
• Document all contacts and interventions accurately per policy.
• Maintain confidentiality and adhere to CareStar policies and procedures.
• Provide assistance with community resource referrals and supports.
