Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• RN or LSW license.
• 1+ year post-degree experience with chronic conditions and/or 3+ years post-degree experience with severe mental illness.
• Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources.
• Effective interpersonal (oral and written) skills to interact professionally with diverse populations.
• Experience with individuals with chronic conditions or severe mental illness.
• Strong knowledge of Medicaid, behavioral health systems, and local community resources.

Responsibilities

• Maintains and builds knowledge of the State and program requirements.
• Maintains and builds knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
• Completes in-home visits to assess eligibility for specified program and conducts safety assessments.
• Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy.
• Provides assistance with community resource referrals as appropriate.
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality as related to patient information.
• Follow the Acceptable UsePolicy while using any information systems owned or controlled by CareStar, Inc.
