Job summary

CareStar, Inc. is seeking an Assessment Specialist to join our Specialized Recovery Support Services program in Trumbull County, Ohio and surrounding areas.

This role conducts in-home visits, assesses eligibility, develops personalized care plans, and connects clients with essential services, while maintaining compliance with program guidelines. The ideal candidate holds a Bachelor's degree in a related field (or RN/LSW) and has experience with chronic conditions or mental illness, Medicaid, and

Qualifications

• Bachelor’s degree or RN/LSW; related field.
• At least 1 year post-degree experience with chronic conditions or 3 years with severe mental illness.
• Knowledge of Medicaid and behavioral health systems.

Responsibilities

• Maintains and builds knowledge of the State and program requirements.
• Maintains and builds on knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
• Completes in-home visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
• Documents all applicant and case-related contacts and interventions in applicant's record per agency and CareStar policy, including standards of content and timeliness.
• Provides assistance with community resource referrals as appropriate.
• Adhere to the CareStar Rule in performance of job responsibilities.
• Understand and comply with CareStar Policies and Procedures.
• Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
