Job summary

CareStar, Inc. is seeking an Assessment Specialist to conduct in-home assessments across Allen County and surrounding areas.

This role develops personalized care plans and connects clients with essential services, delivering compassionate, high-quality care coordination in a mission-driven environment. Ideal candidates hold a related degree or RN/LSW credentials, with experience in chronic conditions or severe mental illness.

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field; or RN/LSW.
• Minimum one-year post-degree experience with chronic conditions or three years with severe mental illness.
• Knowledge of Medicaid and behavioral health systems.
• Strong interpersonal skills with diverse populations.

Responsibilities

• Complete in-home visits to assess eligibility and safety per guidelines.
• Document all applicant contacts and interventions in the record.
• Provide referrals to community resources as appropriate.
• Maintain knowledge of state requirements and program guidelines.
• Adhere to CareStar policies and protect confidentiality.
