Job summary

American Family Care in Norfolk, VA seeks an Advanced Practice Provider (APP) to deliver patient-focused care within defined practice guidelines. The APP will collaborate with other providers and staff to maintain efficient patient flow and high satisfaction across AFC centers that operate seven days a week.

The role emphasizes acute illness/injury care with on-site lab and imaging support. The APP provides detailed histories, diagnostic workups, and management plans, including emergency

Qualifications

• Excellent communicator with staff, patients, and family.
• Professional appearance and attitude.
• ANCC, AANP, or NCCPA Board Certified - (with 1 yr ER/UC experience preferred outside of clinical rotations)
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required
• Able to multi-task and work independently
• DOT Certification preferred
• Able to treat 2 years old and up

Responsibilities

• Perform complete, detailed health histories, review patient records, and order appropriate studies for the patient
• Formulate medical diagnoses and institute therapy or referrals to appropriate facilities
• Institute emergency measures and stabilization in cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions
• Interpret and analyze patient data to determine status and management
• Provide instructions regarding health care and health promotion to patients and families
• Other duties and responsibilities as assigned
