Job summary

Encompass Health in the United States is seeking a Case Manager to connect patients with needed care from admission to post-discharge. You will work with an interdisciplinary team to coordinate treatment plans, oversee discharge planning, and educate patients and families about rehabilitation options.

This role emphasizes timely assessments, documentation, and adherence to regulatory standards while supporting a compassionate, patient‑centered care philosophy across the community you serve.

Qualifications

• Must be licensed to independently complete assessments within your discipline.
• Active license required for the state (Nevada) and practice area.
• CCM or ACM certification must be obtained within two years of entry or already held.
• Minimum 2 years of rehabilitation experience preferred.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Lead discharge planning and contingency planning discussions.
• Educate patients and families on rehabilitation goals and resources.
• Document case management activities and ensure regulatory compliance.
• Participate in utilization reviews and ACT-like meetings as applicable.
