Job summary

Encompass Health is seeking a Case Manager to coordinate patient care from admission to post-discharge and oversee interdisciplinary plans of care. You will manage resources and ensure timely, quality discharge planning while collaborating with a broad care team in a leading national inpatient rehabilitation organization.

The role emphasizes regulatory compliance, patient education, and coordination across departments to optimize patient outcomes and satisfaction.

Qualifications

• Must be qualified to independently complete an assessment within scope of practice.
• Active license if licensure is required in state.
• CCM or ACM certification required OR must be obtained within two years of placement.
• Bachelor's degree required; graduate degree preferred.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience including quality, timeliness, service appropriateness, payors, and expectations.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan appropriate actions.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission; assess goals and complete addendum within 48 hours.
• Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
• Schedule and facilitate family conferences as needed.
• Assist patient with timely planning of resources to avoid discharge delays.
