Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission through post-discharge, guiding an interdisciplinary team to optimize outcomes. You will manage discharge planning, documentation, and communication with families while ensuring regulatory compliance and quality care.

The role emphasizes collaboration, ongoing education, and proactive resource planning to prevent delays and improve patient experiences in a national inpatient rehabilitation setting.

Qualifications

• RN licensure with BSN preferred; diploma acceptable in some states.
• CCM or ACM certification required or must be obtained within two years of entry.
• Two years rehabilitation experience preferred.

Responsibilities

• Coordinate patient care from admission to discharge with the interdisciplinary team.
• Establish tentative discharge plan and contingency plans with the team.
• Complete case management addendums and required documentation.
• Monitor patient experience and ensure quality, timeliness, service appropriateness, and payor compliance.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Educate patients and families on rehabilitation goals and the case management process.
• Assist with timely arranging resources to avoid discharge delays or issues.

Education

Associate Degree in Nursing (RN)

BSN preferred

Bachelor's degree for all other licensed/certified health care professionals

Graduate degree preferred
