Advocate Health in Downers Grove, IL is seeking a licensed Occupational Therapist to evaluate patients referred by physicians, develop individualized treatment plans, and promote independence through therapy and assistive devices.

You will discharge planning with home programs, educate patients and families, collaborate with physicians and care teams, and ensure accurate documentation and compliance with standards.

Qualificações

• Master’s Degree in Occupational Therapy from an accredited program and ongoing CE requirements.
• Licensed Occupational Therapist in Illinois.
• No experience required.

Responsabilidades

• Interpret physician referrals and evaluate patient abilities affecting daily living.
• Establish and implement a care plan with functional goals and appropriate techniques.
• Provide or fabricate assistive devices and monitor outcomes.
• Discharge planning with home programs and family teaching.
• Document care per policy with signatures, progress notes, and billing.
• Promote safety and infection control while using modalities and equipment.
• Supervise OT assistants and students as needed.
