Job summary

Advocate Health is seeking a licensed Occupational Therapist in Wisconsin to evaluate patients, develop care plans, and provide therapeutic interventions to improve daily living activities.

You will establish discharge plans, educate patients and families, and coordinate with physicians and care teams, while maintaining documentation and compliance with insurance guidelines. The role emphasizes patient-centered care and ongoing professional development.

Qualifications

• Bachelor's degree in Occupational Therapy.
• Wisconsin OT license.
• BLS certification within 6 months, if required.
• Excellent communication and collaboration with care teams.

Responsibilities

• Interprets physician referrals and evaluates the patient's abilities and their impact on ADLs.
• Establishes a plan of care and implements treatment; fabricates assistive devices or splints.
• Plans for discharge including home programs and family teaching.
• Completes documentation and applies insurance guidelines for authorization and visits.
• Maintains a neat, safe workspace and performs manual techniques and transfers.
• Uses modalities and exercise equipment with knowledge of indications/contraindications.
• Participates in performance improvement activities and manages caseload.
• Pursues professional growth and orients new staff; participates in evaluations.
• Delivers age-appropriate care across life span per department policies.
