Job summary

Advocate Health is seeking a licensed Occupational Therapist to evaluate a patient’s physical, cognitive, and psychosocial needs and develop individualized plans of care. You will deliver therapy, fabricate devices, monitor outcomes, and coordinate with physicians to ensure optimal patient function and independence.

The role emphasizes patient safety, comprehensive documentation, and ongoing professional growth within a hospital setting.

Qualifications

• Strong communication, interpersonal, and organizational abilities.
• Proficient with basic computer tasks and documentation.
• Ability to assess patients across age groups and needs.
• Demonstrated knowledge of growth and development principles.
• DNV/CARF or similar certifications knowledge may apply.

Responsibilities

• Interprets referrals and evaluates patients’ abilities and impact on ADLs.
• Establishes plan of care and implements treatment based on evaluation results.
• Provides and/or fabricates assistive devices or splints as needed.
• Monitors outcomes and adjusts treatment plans; reports to physicians.
• Completes documentation per policy; handles statistics, billing, scheduling.
• Ensures safety, infection control, and equipment handling.
• May supervise assistants and rehab staff where appropriate.
• Pursues professional development and mentors new staff.
• Drives discharge planning and home program instruction.
