Job summary

Elite Care Management, Inc. is seeking a part-time Home Health Nurse (RN or LPN) to provide one-on-one care to patients in Sleepy Hollow.

You will work with a single patient per shift, plan and assist with home health services tailored to that patient and family, and help improve comfort and well-being during each visit. You will observe patient conditions, administer medications, follow care plans, and maintain detailed documentation while communicating clearly with supervisors and physicians.

Qualifications

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of changes.
• Demonstrate understanding of nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, and monitors effectiveness.
