Elite Care Management, Inc. is seeking a dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to deliver compassionate, one-on-one home health care. This role serves a single patient per shift in Sleepy Hollow, IL, supporting daily living activities and medical needs.

Shifts are Saturdays and Sundays from 10am to 10pm, part-time. You will assess conditions, administer medications, document progress, and collaborate with supervising clinicians to adjust care plans.

Qualifikationen

• Active RN or LPN license.
• Home health care experience preferred.
• Strong communication and documentation skills.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
