Elite Care Management, Inc. is seeking a part-time home health nurse to provide one-on-one care for patients in Sleepy Hollow, IL.

The role focuses on the patient’s comfort, daily living activities, and overall well-being, delivering care in the patient’s home with individualized attention. You will participate in a structured nursing team, following care plans, administering medications, and documenting patient progress.

• Observe patient, record significant conditions and reactions.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the care plan.

Responsabilités

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments, and incidents.
• Administers medications and treatments as prescribed, and monitors effectiveness.
• Manage and direct patient care according to the patient care plan and update as needed.
• Intervenes in response to changes in patient condition and reports to physician or supervisor.
