Job summary

Elite Care Management, Inc. is seeking a dedicated Home Health Nurse (RN or LPN) for part-time shifts in Sleepy Hollow, IL. Each shift is one patient, allowing focused, one-on-one care in the patient’s home.

You will observe and document patient conditions, administer medications as prescribed, and update care plans with the supervising clinician. Strong communication with families and clinicians is essential; requires valid RN/LPN license and CPR certification.

Qualifications

• Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding of organizational nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, and monitors and documents effectiveness.
• Intervenes as applicable in response to changes in patient condition and reports such changes to the appropriate physician or supervisor.
