Job summary

Elite Care Management, Inc. is seeking a part-time Skilled Nurse to provide one-on-one home health care in Sleepy Hollow. You will work with one patient per shift, delivering direct care and coordinating with family to meet daily living needs.

Shifts are Saturdays and Sundays, 10am-10pm. The role emphasizes patient comfort, safety, and adherence to care plans, with paid orientation, weekly pay, and a full benefits package.

Qualifications

• Active Illinois RN or LPN license.
• Current CPR certification.
• TB test or chest X-ray completed within 12 months.
• Driver's license and reliable transportation.
• Social Security Card.

Responsibilities

• Observe and document patient conditions; report changes to supervisor.
• Follow patient care plan and nursing policies.
• Communicate clearly with patient, family, and team.
• Administer medications and therapies as prescribed.
• Respond to changes in patient condition and escalate promptly.

You will get to know your patients and their families and learn about the needs they may have in their home. Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
