Elite Care Management, Inc. is seeking a Licensed Practical Nurse or Registered Nurse to provide one-on-one home health care in Sleepy Hollow. You will work with a single patient per shift, supporting daily living activities and comfort, with a focus on delivering personalized care in the patient’s home.

The role includes comprehensive training, competitive salary, weekly pay, and a full benefits package. Applicants must hold an RN or LPN license and have current CPR and TB testing or equivalent.

Responsibilities

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.

• 10am - 10pm - Saturday / Sunday
