Job summary

Elite Care Management, Inc. is seeking a dedicated Registered Nurse or Licensed Practical Nurse to provide one-on-one home health care for Sleepy Hollow patients.

This part-time role covers weekend shifts (10am to 10pm on Saturday and Sunday), focusing on individual patient care in the home setting. You will assess daily living activities, comfort, and well-being, and implement the patient care plan with close supervision.

Qualifications

• Active RN or LPN state license.
• Strong communication and interpersonal skills.
• Ability to follow care plans and document care accurately.

Responsibilities

• Observe patient and report changes to supervisor or physician.
• Demonstrate understanding of nursing policies and procedures.
• Manage and direct patient care according to the patient care plan and update as needed.
• Administer medications and treatments as prescribed and document effects.
• Respond to changes in patient condition and communicate with the physician or supervisor.

Responsibilities may include the following:

• Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents.
• Demonstrate understanding organizational and nursing policies and procedures.
• Demonstrate strong communication skills, both spoken and written.
• Demonstrate ability to manage and direct patient care according to the patient care plan and update as needed.
• Administers medications and treatments as prescribed, as well as monitors and documents effectiveness of intervention.
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to the appropriate physician or supervisor.
