Family Pillars Homecare, LLC is seeking dedicated RNs and LPNs to provide skilled nursing care in patients' homes across Florida on a per diem basis. You will perform intermittent visits, private duty shifts, and care plan coordination while maintaining high clinical standards.

Florida-licensed nurses with BLS, AHCA background checks, and PT/OT collaboration will thrive in this flexible role, with EMR support and HIPAA-compliant record-keeping.

Qualificações

• Active Florida RN or LPN license (Compact License accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical within past 6 months.

Responsabilidades

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN).
• Uphold infection control standards and agency protocols.

Key Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments
• Carry out shift-based care for patients needing extended clinical support
• Monitor and document patient condition and progress
• Collaborate with physicians and interdisciplinary teams to update care plans
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN)

Experience In Medicare-certified Home Health Strongly Preferred

• Familiarity with EMRs (e.g., Axxess) and knowledge of PDGM & OASIS for RNs
• Excellent clinical judgment, communication skills, and time management
• Reliable transportation and commitment to punctual, professional care
