Job summary

Family Pillars Homecare, LLC is seeking dedicated RNs and LPNs to provide skilled nursing care to patients in their homes across Florida on a per diem basis. The role includes intermittent visits, private duty shifts, and care plan coordination.

Applicants must hold an active Florida RN or LPN license (compact accepted) and be reliable with BLS CPR certification. Experience with PDGM/OASIS is a plus and physician collaboration is essential for updated care plans.

Qualifications

• Active Florida RN or LPN license (Compact License accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical (within past 6 months).

Responsibilities

• Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments.
• Carry out shift-based care for patients needing extended clinical support.
• Monitor and document patient condition and progress.
• Collaborate with physicians and interdisciplinary teams to update care plans.
• Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS documentation required for RN).
• Uphold infection control standards and agency protocols.
