Job summary

Family Pillars Homecare, LLC is seeking dedicated RNs and LPNs to provide skilled nursing care in patients' homes across Florida on a per diem basis. The role includes intermittent visits, private duty shifts, and care plan coordination.

You will perform skilled visits, document care, and collaborate with physicians while maintaining HIPAA-compliant records. OASIS familiarity for RNs is preferred; BLS/CPR and AHCA background screening are required; reliable transportation essential.

Qualifications

• Active Florida RN or LPN license (Compact License accepted).
• Current CPR certification (BLS).
• Level 2 background screening (AHCA-compliant).
• TB screening and physical (within past 6 months).
• Experience in Medicare-certified home health strongly preferred.
• Familiarity with EMRs (e.g., Axxess) and knowledge of PDGM & OASIS for RNs.
• Excellent clinical judgment, communication skills, and time management
• Reliable transportation and commitment to punctual, professional care.
